Have the following ready before you start your application:
- Confirm the owner is a current financial FASA member.
- Confirm the cat is registered with FASA.
- Have previous certificates, challenge or merit awards, judge details and affiliate details ready.
- Arrange for your challenge evidence to be sighted.
Use the current Fees and Charges and the fee displayed in the form when arranging payment.
Which honours level should I choose?
Select the exact title you are applying for. The form includes Champion, Grand Champion, Double Grand Champion, higher Double Grand Champion, Diamond and Companion Cat pathways.
Your selection displays the matching guidance and challenge rows. Check the requirements against the evidence you are submitting.
Choose an Honours Certificate, Certificate + Medallion or Certificate + Rosette. The current fee loads in the application.

Enter the owner's email, FASA member number, name and phone number. Complete the postal address, including suburb, state and postcode.
Check that your contact information is current before submitting.

Use the cat's registered name and registration number. Have the breed, colour, date of birth, sex and breeder details ready.

Select an honours level first to display the matching number of challenge rows.
For each challenge or merit record, enter the date, show, affiliation, and judge and affiliate. Check the displayed guidance and current FASA rules for the required awards, judge count and recognised bodies.

Have the listed challenges sighted by the FASA Honours Recorder or a member of the FASA Management Committee.
The current application also states that Champion and Grand Champion applications must have the previous Honours Certificate sighted. Check with FASA if you need clarification for your application.
Keep copies of previous certificates and official evidence ready for review.
What am I declaring?

The form loads the current fee for your certificate choice. Pay by direct deposit using the bank details shown in the application.
If you have already paid, you may attach a payment receipt and enter your bank-transfer reference. Both are optional. Review the completed application before submitting.

Companion Cat awards apply to eligible domestic or part-pedigree exhibits. Adult companion exhibits must meet the desexing requirements stated in the current Show Rules.
