General form PDFs
- Membership Application or Renewal PDF - Send to the FASA Treasurer; fee $18.00-$25.00 per year.
- Individual Registration / Transfer PDF - FASA Registrar; $10.00 for a first FASA registration or under six months, $15.00 for an existing FASA cat aged six months or over.
- Litter Registration PDF - FASA Registrar; $10.00 for a first FASA litter registration or under six months, $15.00 for an existing FASA litter aged six months or over.
- Prefix Application PDF - Send to the FASA Prefix Registrar; fee $25.00 new prefix, $5.00 dual registration or transfer.
Honours PDFs
Honours - FASA Honours Recorder; $10.00 certificate, $15.00 medallion, $30.00 rosette.
